All pets boarded at Cypress Creek Animal Hospital, P.C., must be currently vaccinated for basic diseases. Required vaccinations for boarding at our facility are as follows:

Dogs:
- Rabies
- DHLPP-C: Distemper, Hepatitis, Leptospirosis, Parvo, Parainfluenza, & Corona
- Bordetella (Kennel Cough)
Cats:
- Rabies
- FVRCP (Rhibotracheitis, Calici, Panleukopenia, and Chlamydia)
- Feline Leukemia is recommended
Proof of current vaccinations will be required, or the clinic may vaccinate the pet upon arrival for boarding.

No one, including us, expects your pet to become ill while boarding, however, in the event that your pet does become ill while at our facility we need instructions on how to properly treat your pet. Please check one of the following boxes

We have your consent to treat your pet as we see necessary if it becomes ill, including any and all measures necessary to treat your pet.
We have your consent to treat your pet only in the event of a life-threatening illness. If treatment becomes necessary, we will use any and all measures necessary to treat your pet.
